Checklist for Hourly Appointment

Information needed to appoint a student:

Hourly Appointment Request Form

Student, Non-Benefits Eligible Application for Employment (PO14) – applicant should complete application and print out

Background Check (Security Sensitive) - applicant should complete top portion of form and print out

I9 Form - applicant should complete top portion of form and print out

Selective Service Form - (UT EID Required) must be completed on line by all male employees age 18 – 25

The supervisor is responsible for having the student complete and signs the above-required paperwork. The Student/Supervisor must bring the completed required paperwork to Wendy Stuessy, Business Services Office, CMA 4.125.

Student needs to complete after appointment is approved:

Paycheck Profile (UT EID Required) applicant will not receive a paycheck until this is completed online

Employee biographical information (UT EID Required) must be completed online

Ethics Statement (UT EID Required) must be completed online

Individual Conflicts of Interest Policy read policy and the statute of law concerning conflicts of interest with three business days

Subchapter C: Standards of conduct and conflict of Interest Provision read policy and the statute of law concerning conflicts of interest with three business days

COMPLIANCE TRAINING MODULES:

CW 101 Introduction to U.T. Compliance Program (UT EID Required) must be completed online

CW 121 Sexual Harassment (UT EID Required) must be completed online

CW 123 Equal Employment Opportunity -EEO (UT EID Required) must be completed online

CW 170 IT Security Awareness (UT EID Required) must be completed online

And any other modules that are required under your UTEID

The Student is responsible for completing the above-required forms and modules online.
